Commercial Performance Drug List

Capital Health Plan encourages the use of generics as the first-line of prescribing. Generic medications are the lowest-cost options to our members and are available at a Tier 1 copay.

If a generic is not available for the condition you wish to treat, the use of a brand name medication found on our Commercial Performance Drug List is preferred. These medications will have a Tier 2 copay. Any brand name not found on this list is considered non-preferred and may result in the highest pharmacy copay for your patient.

Specialty Pharmacy Services for Commercial Members

Effective September 5, 2008, specialty medications must be obtained through Caremark Specialty Pharmacy Services in order to be covered. Capital Health Plan members that are currently receiving a specialty medication will receive a letter instructing them to contact Caremark to initiate enrollment. Caremark will contact the prescriber to obtain a new prescription. For new starts of specialty medications, physicians will need to obtain and complete a Caremark Specialty Enrollment Form (msword 125.00 kB) and, if the drug requires prior authorization, a drug specific Prior Authorization Request Form. Upon completion, the enrollment form should be faxed to Caremark and the Capital Health Plan Care Coordination Department, while the Prior Authorization Request Form should be faxed only to the Capital Health Plan Care Coordination Department.
