New Member Checklist

Step 1.

Choose a primary care physician (PCP) for yourself or a family member. You can use our website to search for a physician. This physician is your primary source for care during office hours and guidance for care when the office is closed. Your PCP is either on call, or has another physician on call, 24 hours a day, 7 days a week.

Step 2.

Transfer your medical records if you are choosing a new doctor. Complete the Authorization to Disclose Protected Health Information Form (pdf 90.06 kB) and give it to the office that now has your medical records.

Step 3.

Register for CHPConnect.external link icon Manage your health care online (create a personal health history, track doctor visits, tests, referrals, diagnoses, and more).

To sign up, have your Member ID number ready.

Learn how to get care as a CHP member. Read:

Find the prescription drugs that you need. Check the Commercial Member formulary page to find which drugs are included on our Performance Drug List.

Step 6.

Find the CHP centers. See the location map (click on the map to download a PDF).

Step 7.

Find where to get lab services using the LabCorp Maps (pdf 109.21 kB) and LabCorp FAQs (pdf 24.75 kB) .

Step 8.

Log into CHPConnectexternal link icon to learn all about your benefits. Consult your Capital Health Plan Member Handbook and endorsements.

Step 9.

Review the Member's Rights & Responsibilities and Privacy Notice.

Step 10.

Complete and mail the Coordination of Benefits Questionnaire (pdf 254.21 kB) within the first 30 days of coverage.
